1) Reflection
1
Reflection is an everyday process
...
Reflective practice facilitates
the development of these skills by fostering and understanding of practice events and
how one’s own approach, personality and personal history contributed to the way
situations arose and how they were dealt with (Oelofsen, 2012; Somerville and Keeling,
2004)
It is important to do reflection in reference to health care as it helps to identify
what happened and what your role in the experience was including your behavior and
thinking and the related emotions
...
If reflective practice is performed comprehensively and honestly, it
will inevitably lead to improved performances

1) Description
I am currently working in emergency as a staff nurse since year 2007
...
There’s one incident which come to my interest on how to deal with this case
...
Confidentiality is a fundamental part of professional practice that protect Human Rights
as it stated in the book of Code of Professional Conduct for Registered Nurses and
Midwives in Brunei Darussalam (2010)
...
45pm, I was
assigned as a triage nurse, I receive a case of 15 years old girl named Diana with
complaint of lower abdominal pain and persistent vomiting for 3 days
...
When I check her urine dip test, I found out that her pregnancy test is positive
...
At that time, Diana was
accompanied by her mother
...
2) Feelings
I feel so shock, distress and disbelief when found out that Diana are positive
pregnant because she is underage and her body structure is smaller when comparing to
other 15 years old girl
...
Diana’s mother looks so worried and
wondering what had happened to her teenage girl
...
It’s very
difficult for me to find words on how to communicate in sensitive manner taking account
of all the needs of the girl, this is because Diana looks very afraid, insecure and she tell
me that her mother is a very uneasy person to discuss with
...
Desiree (2012) notes that although both Asian and Western bioethics stress
truth-telling and autonomy, the Asian conception of autonomy considers the family as
the autonomous unit whereas the Western application of the ethical principle in
medicine generally refers to the individual
...
(2
...
I bring her to a quiet room and said because of Diana is still underage, I need
to explain this matter with her mother, because I’m afraid if she’s the one who will tell
this news to her mother, her mother may not able to handle calmly the news
...
By that she not only focuses on one thing, but for safety of her daughter
too
...
6
According to Turner (2003) young children reported a lack of knowledge of
services available from professional
...
2002, Hayter 2005) as stated by Coad and Shaw (2008)
...
When I tell Diana’s mother about what really happen to her daughter, she’s burst in
tears, very upset and feel that her daughter has betrayed her trust
...
The bad about the experience are lack of time is often blamed for limiting good
communication, because only two nurses assigned in triage area, and a lot of patient is
still waiting to be triage makes me have limited time to spend with Diana and her
mother
...
The good about the experience
are truth telling, at least Diana’s mother knows what actually happen to her daughter, I
bring them together in a room and break the news gently to her mother and Diana feels
a bit secure when I’m on behalf of her breaking this news to her mother
...
4) Analysis
7
At that time, I felt that I’m not completely help to relieve the girl and her mother
emotion and relationship, because after I’m breaking the news and treatment option the
I excuse myself to inform the gynae ward about the girl admission, leaving the mother
and her daughter sulking in sadness with the unpleasant news
...
I feel that I should be train or have a pediatric nurse with me who is highly
train in dealing with this situation because usually we staff nurse talk and explain to
patient or family based on our past clinical experience but not on what we are train for
...
2005)
(2
...
I should be
more supportive to Diana and her mother, helping them as much as I can
...
And most health care staff acknowledges that they were
not adequately equipped with knowledge and skills to provide high quality in caring a
teenage girl with pregnancy
...
(2
...
Having a breaking bad news
handbook for staff to patients and their family
...
At least two staff nurses assigned in every working shift has
fully trained as a pediatric nurse
...
Be more sensitive, ask them politely if there’s anything I can do, and give them
more time to decide
...
Allow
and encourage the person to express her feelings freely, example by crying, talking
about concerns, fears, anger, anxieties and etc
...
Generally, all nurses love to give
good news, and therefore breaking bad news is very hard for them
...

CHAPTER 4
4) RECOMMENDATIONS
In Emergency Department, there should be more nurses to be train as a
paediatric nurse
...
Facilitate privacy; give the person the bad
news in a place that is quiet and private
...
Avoid physical barriers, such as a desk between you and the
person
...
Allow enough uninterrupted time
during the initial meeting for the patient or her mother to think about what you are telling
her so that she can discuss it with you and ask you question
...
Being confronted with the bad news is a potentially
stressful experience for the patient or her mother
