DRUG STUDY
NAME: RIE NICOLE R
...
A
...
A
...

Theophylline competitively
inhibits type III and type IV
phosphodiesterase (PDE),
the enzyme responsible for
breaking down cyclic
AMP in smooth muscle
cells, possibly resulting in
bronchodilation
...
In
inflammatory states,
theophylline activates
histone deacetylase to
prevent transcription of
inflammatory genes that
require the acetylation of
histones for transcription to
begin
...
It
works by relaxing the
muscle around the
airways in the lungs
...

Theophylline has few
outright contraindications
for use other than a history
of hypersensitivity to
theophylline or any inactive
ingredient in the drug
...

Potentially Fatal:
Convulsions, cardiac
arrhythmias,
hypotension and
sudden death after too
rapid IV injection
...

texture, lesions; reflexes,
bilateral grip strength,
A very serious allergic reaction
affect; P, auscultation, BP,
to this drug is rare
...

• Interventions

Caution patient not to
chew or crush entericcoated timed-release
preparations
...

Do not give timed-release
preparations with food;

400 mg/24 hours; 450 mg;
600 mg/24 hours)

these should be given on
an empty stomach, 1 hr
before or 2 hr after meals
...

WARNING:
Monitor results of serum
theophylline level determinations
carefully, and reduce dosage if
serum levels exceed therapeutic
range of 10–15 mcg/mL
...

WARNING:
Keep diazepam readily
available to treat seizures
...
If a timedrelease product is
prescribed, take it on an
empty stomach, 1 hour
before or 2 hours after
meals
...

• Avoid excessive intake of
coffee, tea, cocoa, cola
beverages, and
chocolate
...

Smoking cigarettes or
other tobacco products
may markedly influence
the effects of theophylline
...
Notify
your health care provider
if you change your
smoking habits while you
are taking this drug; it may
be necessary to change
your drug dosage
...

Do not use this drug during
pregnancy; using barrier
contraceptives is advised
...

Report nausea, vomiting,
severe GI pain,
restlessness, seizures,
irregular heartbeat
...

Inhalation
Each actuation of aerosol
dispenser delivers 90 mcg
albuterol; 2 inhalations q
4–6 hr; some patients may
require only 1 inhalation q
4 hr; more frequent
administration or larger

MECHANISM OF
ACTION
The pharmacologic
effects of beta-adrenergic
agonist drugs, including
albuterol, are attributable
to stimulation through
beta-adrenergic receptors
of intracellular adenyl
cyclase, the enzyme that
catalyzes the conversion
of adenosine triphosphate
(ATP) to cyclic-3’, 5’adenosine
monophosphate (cyclic
AMP)
...

INDICATIONS
Relief and prevention of
bronchospasm in
patients with reversible
obstructive airway
disease
Inhalation: Treatment of
acute attacks of
bronchospasm
Prevention of exerciseinduced bronchospasm
Unlabeled use: Adjunct
in treating serious
hyperkalemia in dialysis
patients; seems to lower
potassium
concentrations when
inhaled by patients on
hemodialysis

CONTRAINDICATIONS ADVERSE EFFECTS

Contraindicated with
hypersensitivity to albuterol;
tachyarrhythmias,
tachycardia caused by
digitalis intoxication;
general anesthesia with
halogenated hydrocarbons
or cyclopropane (these
sensitize the myocardium to
catecholamines); unstable
vasomotor system disorders;
hypertension; coronary
insufficiency, CAD; history of
CVA; COPD patients with
degenerative heart
disease
